Mr MCCLUGGAGE had a number of health conditions and lived in sheltered accommodation. He had not been seen by staff since late April 2025. He was taken to hospital on the 6th May 2025 when he was found after intervention by his brother to be very unwell and stuck in his chair for a number of days. He was taken to hospital where despite treatment he died. |

| 27/04/2026 | 28/04/2026 | SHAW, Julie | Julie SHAW died on the 28th October 2025 at her home address. Ms Shaw suffered a ruptured abdominal aortic aneurysm in February 2025 which was operated upon. Following the surgery she suffered a number of complications which resulted in loss of blood flow to her bowel and required many more surgeries. In the months that followed, Ms SHAW endured a long hospital stay where she struggled to manage adequate oral intake and was told she was not eligible for inpatient mental health support. She was discharged with a package of care but her family provided the majority of her care. Ms SHAW had been unable to maintain adequate oral intake for many months and her health continued to deteriorate. She received support from palliative care services before her death. |

Mrs BAKER had a number of underlying health conditions and lived in a care home. She developed a pressure injury to her sacrum which became infected and caused her death. |

| 30/04/2026 | 30/05/2026 | ALBIN, Melvin John | Melvin John ALBIN died on the 31st July 2024 at the Royal Preston Hospital. Mr ALBIN had a number of underlying health conditions but was awaiting discharge when he was taken unaccompanied for an x-ray. Mr ALBIN required an escort for his safety. Whilst waiting unaccompanied to be returned to the ward, Mr ALBIN attempted to stand and fell, causing injuries which included a small bleed in his brain. Following the fall Mr Albin's consciousness levels deteriorated and he developed aspiration pneumonia which caused his death. |

| 01/05/2026 | 01/05/2026 | WARWICK, Lillian | Lillian WARWICK died on the 22nd February 2026 at the Royal Blackburn Hospital. Mrs WARWICK was found on the 14th February 2026 at the bottom of her stairs following a fall. She declined to attend hospital at that time. She attended her GP on the 18th February who was arranging an outpatient CT scan. However, later that evening she became unresponsive and was taken to hospital where scans indicated she had suffered significant bleeding within her brain. She was not able to benefit from treatment for the bleeding and her care was re-orientated to comfort care prior to her death. It is not possible to determine the circumstances of Mrs WARWICK's fall and whether an accident or medical event caused her to fall. |

Whilst on her staircase, she had secured a bedsheet around her neck and consumed a possibly fatal quantity of alcohol before sliding down the stairs. | 05/05/2026 | 05/05/2026 | MANSER, Diane Veronica | Diane Veronica MANSER died on the 8th November 2025 at the Royal Blackburn Hospital. Mrs MANSER was an inpatient being treated for ineffective exacerbation of longstanding chronic obstructive pulmonary disease when she developed a spontaneous retroperitoneal haemorrhage. Mrs MANSER had been treated with blood thinners due to concerns about pulmonary embolism. The bleeding was treated conservatively with blood transfusions but despite improved blood markers, Mrs MANSER continued to deteriorate. The development of the bleed contributed to Mrs MANSER's inability to overcome the infective exacerbation of chronic obstructive pulmonary disease and thus contributed to her death occurring. |

Mrs WRATHALL had a medical history which included advanced dementia and she lived in a care home. On the 10th January 2026 she had an unwitnessed fall which resulted in a fractured neck of femur. Mrs WRATHALL underwent surgery to repair her femur on the 12th of January 2026 but suffered bone cement implantation syndrome, a known risk of the surgery and died. It is not possible to determine whether an accident or medical difficulty caused Mrs WRATHALL to fall. |
