08/01/2026 20/01/2026 PATTISON, Lee Steven Lee Steven PATTISON died on 21 January 2020 at 2 Mannin Way, Lancaster. Mr PATTISON died due to severe blunt force head injury sustained when he fell from an approximately ten metre height. Mr PATTISON was working as a labourer using a scissor lift machine, when the platform of the scissor lift tipped, causing him to fall. Investigations confirmed that only two of the ten screws required to hold the platform to the bracket of the scissor lift were in place at the time. The absence of the appropriate number of screws caused the platform to fall. The scissor lift had been re-assembled following re-painting. During the re-assembly the screws attaching the platform to the bracket of the scissor lift had not achieved the required tension. Insufficient tensioning of the screws allowed them to become loose. The design of the platform mounting joint did not prevent the screws from loosening. The inspection regime, which was performed in accordance to guidance, did not identify that screws were loose or missing before the platform was used by Mr PATTISON.

14/01/2026 14/01/2026 WEBSTER, Theo Simon Daniel Theo Simon Daniel WEBSTER died on the 22nd May 2025 at Blackpool hospital following a deliberate overdose of propranolol. Theo did not intend to cause his death by his action which was in the context of struggles with ADHD and long term drug use which likely contributed to his impulsive actions.

16/01/2026 16/01/2026 MCKENZIE, Alison Louise Died from a known complication of surgery. 
16/01/2026 16/01/2026 NAISH, David John David John NAISH died at Royal Lancaster Infirmary on the 19th of July 2025 from an infection. He had suffered a fall at his home on the 6th of June 2025 fracturing his left femur which required surgery undertaken on the 9th of June 2025. In hospital the vulnerability of his skin breakdown was not recognised or escalated appropriately yet was unlikely to have affected the outcome. 
20/01/2026 20/01/2026 FERGUSON, Patricia Irene Patricia FERGUSON died on the 14th December 2024 in Blackburn hospital from a unsurvivable brain haemorrhage. Mrs FERGUSON was admitted to hospital on the 8th December 2024 and was diagnosed and treated for suspected bacterial endocarditis following mitral valve replacement in 2011. Mrs FERGUSON deteriorated on the 13th December 2024 and was diagnosed with a significant brain haemorrhage which is a known complication of endocarditis. Her death was contributed to by the incorrect administration of enoxaparin.

21/01/2026 21/01/2026 O'BRIEN, Brody On the 7th October 2025 Brody O'BRIEN died at Whitworth Mill on Market Street in Rochdale from hanging. He had suspended himself from a rafter in the derelict mill using a ligature to the neck. He did not intend to cause his death in doing so.

22/01/2026 22/01/2026 STATHAM, Sheila Mary Sheila Mary STATHAM who had become increasingly frail and suffered from a number of comorbidities died at her home on the 9th of October 2025 from an infection following revision hip surgery in October 2022

22/01/2026 22/01/2026 WALKER, Emily Rose Emily Rose WALKER Died on the 6th January 2025 at the St Catherine's Hospice, Lostock Lane, Preston. Emily had been diagnosed with ovarian cancer in 2024 and despite surgery and chemotherapy had suffered recurrence and spread of a highly aggressive cancer. Emily underwent a venting gastrostomy procedure on the 29th November 2024 with a view to symptom management but her liver was injured in the course of the process which resulted in an infection. Emily continued active treatment for the infection and symptomatic treatment which enabled her to attend her own wedding and spend Christmas at home with her family. She was placed on end of life care on the 30th December 2024. Emily's death was caused by her aggressive cancer. If she had not undergone the venting gastrostomy procedure, it is more likely than not that she would have died sooner than she did. However the procedure resulted in an infection which contributed to her death when it did occur.
