If you have a long term condition (a condition that lasts a year or longer and has a big impact on your life), you may require ongoing care and support.

The NHS long-term conditions assessment provides a guide to the help and support you may be entitled to.
If your needs are mostly health related the NHS continuing healthcare is responsible for supporting you.
We understand you may not want to ask us for our support. You may have read our assessment and eligibility criteria and feel you are not eligible for our services. If so, other options for arranging your own care are available. If you have tried these options or feel you need our support find out about getting our support.
Help to find a job or stay in work
If you have autism or a long term condition that makes it difficult to find a job the employment support service may be able to help if you:
- are working age
- have a goal of 16 hours or more paid employment per week
