We want to make it as easy as possible for you to find the right support.
You can get help and arrange your own care or care for friends and family without having to contact us.
If you think you need our help you can complete a self assessment to find out if you are eligible to receive social care support from us. You can also get advice about equipment to make daily living easier.
Request for care and support
If you think adult social care could support you, a family member or friend you can request a referral.
This will make it easier for us to consider your needs and see how we can support you.
We may connect you to an organisation, such as a voluntary, faith or community group, or refer you for a full needs assessment if we think you may need social care support.
