Person centred support plan training - residential services- OP0005
Who is this course for?
This one-day training is for all the care and management staff in the Residential Teams only working within the Older People Service
Aims
To give an awareness of using person centred thinking and approaches to help the people you support have more choice and control in their lives. For staff to provide the best support they can in ways that reflect what is important to the person.
Please note that delegates must bring a copy of their own 1 Page Profile to the session (even if it is not fully completed) and at least one resident's 1 Page Profile but can bring more than one.
Objectives
At the end of the course participants will be able to:
• Express a better understanding of person-centred thinking
• Write better quality support plans
• Identify a range of person-centred tools
• Use person centred tools effectively
• Recognise the person at the centre of the support plan
• Identify family members and friends in developing the support plan
• Recognise their own role and how they fit into completing a qualify support plan
• Explain the importance of planning for end of life

For virtual sessions staff will need their own laptop/PC or smart device to log on to the session.
