SEND professional portal
The professional portal is a way for agencies such as health, social care and educational settings to complete SEND forms online.
The Inclusion Service (previously the SEND Service) will use the professional portal to delegate forms to agencies. You will get an email to notify you if you have a form to complete.
We will be using this for:
- Educational psychology consultation meeting requests
- Education, health and care needs assessment requests
- Education, health and care plan annual reviews
Using the professional portal
Register or log in
If you need to fill in a form on the professional portal you will get an email with a link to the portal.
The first time you use the professional portal you will need to register for a new account.
As part of the login process you will get an authentication code by email, which you will need to enter after your email and password, for extra security. We recommend you copy and paste this. You will need to do this each time you log in.
Saving forms
You can save an incomplete form if you need more time to complete it.
Before submitting, we recommend that you use the PDF button to save as PDF if you need a copy of the information as it will not be available after you submit it.
Guidance for educational settings
Education, health and care needs assessment requests
When you are considering submitting a request for an EHC needs assessment contact your allocated SENDO and request attendance at a TAF meeting.
The SENDO will attend the meeting and support you to review the documentation that you wish to submit.
If the SENDO agrees that it would be appropriate to submit a request for an EHC needs assessment, you will then need to complete the EHC Request form and One Page Profile via the professional portal. The form will be available for you to complete for two months.

Education, health and care plan annual reviews
We are currently developing the annual review process with a small number of pilot schools.
