Request adult social care support
If you think adult social care can help support a patient or client you can request a referral.
If the patient or client is under 18, request support from Childrens' Services.
If you are concerned about an adults safety, report a concern about a vulnerable adult.
If your concern is for an assessment under the Mental Health Act, request a referral to the Approved Mental Health Professional Service (AMHP).
Before you start
The referral will help us decide what support the patient needs or other options available.
We will need to know:
- your personal details, such as name, and address
- personal details of the patient, such as name, date of birth and address
- next of kin details of the patient
- support the patient receives from other people
- any mobility issues the patient has
- GP details
- aids/adaptations used by the patient
- medical conditions or medication
Completing the referral form
You must have an email address and register for an adult social care account to complete a referral.
We will send a security authentication code to your email address when you register.
After you have registered, you can complete and submit the referral form, or save and come back to it.
We will email you a different security authentication code each time you sign in to your account. The code is valid for 10 minutes and can only be used once.

What happens next
We will review your request and let you know what happens next. We will contact the patient to discuss the support available to them.
