Request adult social care support

If you think adult social care can help support you, a family member or friend you can complete our referral form.

Before you start

The information you give us will help us consider the options available to you and work with you to ensure the right support is in place.

This could be support provided by us, one of our partner organisations such as a care provider or by a community, voluntary or faith group.

We will need to know:

  • personal details, such as your name, date of birth and address
  • next of kin details
  • support you receive from other people
  • any mobility issues you have
  • GP details
  • aids/adaptations you use to help get around
  • medical conditions or medication

Completing the referral form

You must have an email address and register for an adult social care account to complete a referral.

We will send a security authentication code to your email address when you register.

After you have registered, you can complete and submit the referral form, or save and come back to it.

We will email you a different security authentication code each time you sign in to your account. The code is valid for 10 minutes and can only be used once.

If you need support that relates to a visual or hearing impairment, telephone 0300 123 6720 to allow us to go through questions specific to your needs.

What happens next

We will review your request as quickly as possible and let you know what happens next.

We will consider the best care options for you.

If you are not eligible to receive social care support from us, we may recommend local services and suggest ways you might find funding to pay for them.

If we don't think a full assessment is necessary, we may discuss other options available to you. This might be a referral to occupational therapy, Technology Enabled Care or other services.

If we think you need a full care needs assessment, we will work with you to put the best social care support in place to support you to live as independently as possible.
