SEN support plans (SSP) and education health and care (EHC) plans and preparing for adulthood
If you have an SEN support plan (SSP) or an education health and care plan (EHC plan) your annual reviews will include a discussion about preparing for adulthood.
Annual reviews
Your school or college will set up a review of your plan every year, called an annual review.
You'll have the chance to talk about whether or not the help you're getting is working for you. You can also think about your future like what you want to study and what job you would like.
This is a chance to talk about more than your educational needs. You will start to think about things such as:
- health needs
- personal and social development
- training and employment
- transport and independent travel
- housing
- leisure
Preparing for adulthood (PfA) outcomes
From year 9 your SSP or an EHC plan will focus on preparing for adulthood. This should include outcomes that you would like to achieve by the time you are an adult.
They should be achievable and should be based on your aspirations, interests and needs.
Planning will take account of what support you need to achieve these outcomes. Progress towards your outcomes will be reviewed each year.
The outcomes are grouped into these types:
Employment or occupation (jobs)
Your plan will help you think about what kind of job would be right for you as an adult. The plan will help you develop the skills you need.
Independent living
Your plan will help you develop your independence. This will aim to give you the right amount of choice and control about where you live and the support you get.
Participating in society
Your plan should help you:
- develop your interests
- take part in the local community
- make friends and relationships
Health and wellbeing
Your plan will help you develop your physical and emotional health and wellbeing as you move into adulthood.
