There are several ways you can order a copy birth certificate:

• In person at the above address 9.00am - 4.00pm Monday - Friday (10.30am first Wednesday every month)

  Please be aware when applying in person as there may be a waiting time whilst the certificate is being produced

Cheques or postal orders should be made payable to Lancashire County Council.

Standard Birth Certificate
This certificate will give a complete copy of the birth entry to include name at birth, date of birth, place of birth and any parent details.
Required for: passports and other legal purposes.
Cost: £9.00

Short Birth Certificate
This certificate will show only the name and surname, sex, date of birth and district of birth.
Cost: £9.00

Adoption Certificates
Records of Adoptions in England and Wales are held by The Registrar General.

You must have the following information to obtain a copy birth certificate:

• Full Name at Birth
• Date of birth
• Place of Birth - (full address or name of Hospital)
• Fathers full name
• Mothers full name at time of childs birth - (including maiden name)
